Medical & Event Forms

Attention Parents!

Before your student can come on an overnight off campus event (such as District Youth Gathering, Revolve, NYG, Lock-In's, etc), they need to have the following Medical Release Form turned in to me. This form lasts from 2008-2011, but must be updated and resigned every year. If you have one on file, but have no updated it for 2009, please contact DYFM Annie Tiberg ASAP. If you do not have one, please print this out, fill it out, and turn it in to Annie prior to participating in any off campus overnight youth event.

For off campus 1 day events (such as Lazer Tag, service projects, etc), the Event Release Form is needed. This is kept on file for that one event and then shredded to protect privacy.  If your student intends to come to more events, the Medical Release Form is encouraged, so that we have some history on file.
